Nurse Practitioner, Matthew 25 (Fort Wayne, IN)

Nurse Practitioner – 36 hours (full time) 

Matthew 25 is accepting applications for a full-time Family Nurse Practitioner to provide professional healthcare needs to the underserved, uninsured individuals in Northeast Indiana and Northwest Ohio. This position reports to the Medical Director. 

This position requires working with patients, diagnosing and treating acute, episodic, or chronic illness, conducting exams independently or as part of a healthcare team. May focus on health promotion and disease prevention. May order, perform, or interpret diagnostic tests such as lab work and x-rays. May prescribe medication. Specific duties include: Analyzing and interpreting patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses. Developing treatment plans based on scientific rationale, standards of care, and professional practice guidelines. Diagnosing or treating acute healthcare problems such as illnesses, infections, or injuries. Prescribing medications based on efficacy, safety, and cost as legally authorized. Counseling patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter (OTC) medications, or herbal remedies. Recommending interventions to modify behavior associated with health risks. Detecting and responding to adverse drug reactions. Educating patients about self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances. Diagnosing or treating chronic health care problems such as high blood pressure and diabetes. Maintaining complete and detailed records of patients' health care plans and prognoses. Providing patients with information needed to promote health, reduce risk factors, or prevent disease or disability. Performing routine or annual physical examinations. Treating or referring patients for primary care conditions. 

Must be a registered nurse that has specialized graduate education; Graduate of an accredited program required. Professional experience preferred. Experience with electronic medical records preferred. CPR Certified. Bilingual preferred. 

Position is considered full-time requiring 36 hours per week and includes benefits. General schedule is Monday, Tuesday, Wednesday, and Friday 8:30am-4:30pm; Thursday 8:30am-12:30pm. May require some evenings; however, no weekends or major US holidays. Position will remain posted until filled.
